To date, the vast majority of infections of Swine Flu in humans have been acquired by close contact with domestic poultry. However, there is one documented case of human-to-human transmission in Thailand. It remains a curious fact that most people who have worked with poultry infected with the bird-flu virus, culling infected birds for example, have not become infected-a fact that no one seems able to adequately explain.
The antigenic changes (that is, changes in the exterior proteins-Hemagglutinin and Neuraminidase) in bird and human influenza viruses go on year after year. Annual influenza outbreaks typically do not cause severe illness and death, because people have built up natural immunity to the viruses over a period of years. Rapid spread of H5N1 viruses between humans, on the other hand, will led to explosive global expansion in which everyone is at risk for infection, illness, and death because of a complete lack of natural immunity to the virus.
Because avian influenza or “bird flu?is much more deadly than the regular annual flu in humans, coupled with the high infectiousness of influenza viruses in general, a bird-flu pandemic is potentially very deadly, causing many tens-or even hundreds-of millions of deaths across the globe. In September 2005, Dr. David Nabarro of the World Health Organization (WHO) stated, “A new pandemic would claim between 5 million and 150 million people.?
Once a flu pandemic starts, everyone will be at risk of getting pandemic flu, because no one has natural immunity to the virus. However, certain groups may be at greater risk of dying than others. There are strong indications that the coming bird-flu pandemic may be similar to the influenza pandemic of 1918 and, if so, healthy adults would be most at risk of dying, along with pregnant women and the elderly.
Why are healthy adults at greatest risk? It appears that the H5N1 bird-flu virus causes a massive immunological response against the virus in those with the strongest immune systems. Unfortunately, this causes the release of human enzymes called “cytokines,?which destroy lung cells along with viral particles. This, in turn, causes a deadly outpouring of fluids into the lung, which interferes with the exchange of oxygen and carbon dioxide. This reaction is frequently fatal. There are few effective treatments-one of the only treatments available is to place the patient on a ventilator, a mechanical respirator. Even with this treatment, patients often die from complications.

An Overview of Swine Flu
Swine flu or influenza is a condition caused by strains of subtypes of Influenza A virus called H1N1, H1N2, H3N1, H3N2, and H3N3. These viruses are common in pigs located in midwestern United States, Canada, South America, Mexico, Mainland China, Japan, Taiwan, Europe, and other Eastern Asian countries.
Transmission of influenza virus from swine to humans is relatively uncommon and does not always result to human influenza but often leads to production of antibodies in the blood. When properly cooked, pig meat does not have the potential of passing the virus. Transmission that leads to human influenza is called zoonotic swine flu.
People who work with pigs, particularly those with direct exposure, are at greater risk of being infected with swine flu. Towards the middle of the 20th century, identification of influenza subtypes became likely paving the way for an accurate diagnosis of transmission to humans. Since then, 50 infections have been confirmed and recorded.
It is rare for these strains of swine flu to be transmitted from one human to another. In humans, the symptoms of swine flu are similar to common influenza as well as influenza-like illness such as chills, sore throat, fever, coughing, muscular pains, severe headache, and general discomfort.
The 2009 swine flu outbreak in humans was caused by a new strain of influenza A virus subtype H1N1 which have genes that closely resemble swine influenza. The root of this new strain is not known. According to the World Health Organization for Animal Health, this new strain has not been isolated in pigs. It is capable of human-to-human transmission and manifests the normal symptoms of influenza.
Swine can be infected with human influenza such as the case of the 1918 flu pandemic and the 2009 flu outbreak. Swine flu was first proposed as a disease associated with humans during the 1918 flu pandemic. During that time, pigs became simultaneously sick with humans.
Influenza virus as a cause of disease in pigs was first identified in 1930. For the next six decades, strains of swine influenza became almost exclusively H1N1. From 1997 to 2002, new strains of three varied subtypes and 5 different genotypes were identified as the cause of influenza among pigs in North America.
From 1997-1998, H3N2 strains developed. They included genes acquired through reassortment of human, avian, and swine viruses and have been a principal cause of wine influenza in North America. Reassortment between H1N1 and H3N2 resulted to the development of H1N2. In Canada, a strain of H4N6 resulted from the reassortment of avian and swine flu but was isolated on a single farm.
The H1N1 variant of swine flu is one of the descendants of the strain that brought about the 1918 flu pandemic. While persisting in pigs, the descendants of the 1918 virus have also been transmitted throughout the 20th century which resulted to the usual seasonal influenza outbreaks.
It is interesting to note that direct infection from pigs to humans is rare, with only 12 confirmed cases in the United States since 2005.
However, since the influenza strains remain in the pigs after they have disappeared in the human population can make these pigs a reservoir where the influenza virus could survive and later on transmitted to humans as soon as their immunity to the strain is no longe effective.

When Swine Flu hit most Asian countries, it prompted the World Health Organization (WHO) to urge China to test its wild birds, particularly geese that migrate from its waters during the harsh winter. The disease spread so fast from one bird to another that about 5,000 birds were killed in one season. Although many are divided on the possibility of direct human to human transmission of the virus, this possibility has not been ruled out. Viruses, in general, are known to mutate. In the case of Swine Flu, family members infected with it may show different severity of symptoms, prompting a misdiagnosis. Also, if a person who has the common flu becomes infected with Swine Flu at the same time, it can lead to the mutation of the Swine Flu virus.
At the University of Leicester in Great Britain, it was discovered that a full-blown Swine Flu pandemic could cause an 80% mortality rate. A team led by Karl Nicholson is developing the Swine Flu vaccine with the goal of decreasing the fatality should a fourth major pandemic occur. In the last century, there have been three recorded major pandemics: the Spanish Flu in 1918, the Asian Flu in 1957 and the Hong Kong Flu in 1968. In total, the three pandemics claimed at least 20 million people.

It is important, however, to avoid raw birds and other raw markets meats from being contaminated. Cook eggs properly and check that the yolks are not runny. The simple act of washing your hands thoroughly with soap and water after handling raw meats could spell the difference. Keep in mind that transmission of the Swine Flu virus to humans usually happens during the slaughtering process and handling of infected Swine Fluids. After slaughtering infected poultry, the virus typically stays in the intestinal and respiratory tracts, not in the meat itself. Cooking at right temperatures can help avoid the virus from spreading.

Information is being disseminated in order to make people aware of the Swine Flu, its effects and what to do to avoid getting infected.
Viruses are constantly mutating and evolving. Health watchers, practitioners and scientists are concerned about this because if a pandemic occurs, there won’t be enough time to prepare and develop a vaccine. They fear that we are once again on the brink of another major pandemic threat. However, with the strides being made by technology every day, hopefully the casualty won’t be nearly as high as the casualty of the past three major pandemics that claimed at least 20 million lives worldwide.
Recalling the 1976 Swine Flu Debacle
In the history of the United States, this is not the first time that the country is experiencing a swine flu outbreak. The first recorded incident of H1N1 infection in the US was recorded on January 27, 1976, when there was a small outbreak of mild respiratory illnesses took place at Fort Dix Army Base in New Jersey.
According to throat cultures obtained from sick soldiers, each of the patients were infected by “swine-like flu virus” which have been unknown to humans since 1930. It was believed that the same virus was also responsible for the worst flu pandemic in the United States in 1918-1919 which led to the death of half a million Americans.
To many people, the discovery of the 1976 was more of a debacle than a victory because after only 10 weeks of implementation, vaccination efforts came to a premature close as the program led to complications which were associated to the shots.
After the death of Private David Lewis after participating in a forced five-mile march the night before his demise, Dr. David Sencer and his colleagues attributed the death to strains of swine-like flu virus. Upon the advice of specialists across the United States, Dr. Sencer called on then President Gerald Ford to launch a nationwide mass inoculation.
President Ford and the US Congress heeded the call and in October implemented the $137-million National Inoculation Program. However, after only several days of implementation, there were reports that the vaccine being used for the program made the patients prone to Guillain-Barre Syndrome, a rare neurological disorder which results to temporary paralysis but could be risky.
Prior to its premature end in December 1976, the vaccine was administered to over 40 million Americans comprising almost 25% of the population. From the over 500 people who experienced Guillain-Barre Syndrome after vaccine administration, 25 people died. As a result, the Federal Government paid millions worth of damages to the families of the victims.
Meanwhile, the epidemic which some experts predicted would affect 50 to 60 million Americans during that time never happened. According to the Center for Disease Control, there were only 200 confirmed cases and one death.
It is understandable why the current H1N1 outbreak has been a source of anxiety for the public who recalls the 1976 event. It serves as a lesson for the government and health officials who needs to make a decision on the current swine flu in the days and weeks to come.
Dr. David Sencer, who is now retired and resides in Atlanta, believes that the 1976 debacle brought good things as well as bad. The belief that the 1918-19 flu epidemic resulted from swine-like virus partly contributed to the 1976 setback.
Although current studies have shown that the cause of the epidemic was bird flu, it did not ease the current anxiety prevailing in the public. The 1918-19 epidemic led to the death of 500,000 people in the United States and more than 50 million worldwide.
In Mexico, where the 2009 swine flu outbreak, originated, 22 people have died from the most recent outbreak. Government officials have been under fire for the way they have handled the situation. However, with such a mystery surrounding the threat, Dr. Peter Katona, who is an authority on infectious disease from UCLA, believes that people have failed to understand the challenges posed by such problem.
You And The Swine Flu: Knowledge That May Save You
With this new strain of influenza running around, properly called Type A H1N1 swine influenza, but more popularly known as the swine flu, I think it would be nice to give people the 411 on the latest medical emergency that’s got the world’s attention.
Swine influenza came from pigs, hence the name, and pigs usually have regular outbreaks of this disease. The problem with it this time is that this strain of influenza has managed to jump the species and went over to us Homo sapiens and cased a bit of havoc.
Pigs don’t have to worry about it because most of them have built-in resistances to the diseases, mostly because of the constant evolutionary exposure to the disease. They like us when we receive the flu: a few chills, sniffles and a bit of bed rest and we’re right as rain. Problem with that is us humans don’t have this resistance to swine flu, mostly because we’re not pigs. The pigs would be in the same boat if a human strain had jumped into the porcine population.
The other problem with it is its virulence. The Center For Disease Control have determined that this strain of swine flu is contagious and can be passed to other people through the normal vectors: the virus can be transmitted when someone touches something that is contaminated and the puts it in his eyes, nose, or mout. It’s even airborne as microscopic droplets can travel through the air when someone sneezes. The CDC, however, is still a bit hazy on some other factors like incubation time and much contact is too much contact.
Another problem is that the swine flu has the same symptoms as regular human flu, just worse by an order of magnitude. High fever, cough, sore throat, body aches, headaches, chills and fatigue can either mean you’ve been hit by the ordinary human flu and should take the usual cures or maybe you have the swine flu and need to go to the doctor immediately. Personally, it would be better to take no chances and just go to the doctor. Diarrhea and vomiting are the big warning signs though. Death by flue isn’t direct though. It just compounds with other diseases like pneumonia and makes it even worse. It can also kick any existing medical conditions you have like asthma up a notch.
So, the question on your mind now probably is: is there no hope or should I just run to the hills to avoid human contact? Thankfully, you don’t have to go that far. The CDC has recommended the use of several medicines to treat yourself with and to prevent the spread of the disease. Oseltamivir and zanamivir are viral inhibitors that make sure that the virus does not reproduce. More common antiviral drugs that are bought over the counter can also be effective. They make the symptoms milder and help your body heal itself faster. They can also stop some of the higher level complications that can happen when you are infected, if you take them early enough.
Of course, an ounce of prevention is better than a pound of cure. To avoid getting infected, try to avoid close contact with sick people and wash your hands on a regular basis, especially before you eat. If you have the unfrotunate luck of getting sick, isolate yourself and check your symptoms. Immediately consult with your doctor if your sickness persists more than usual.
The swine flu is just the latest in a long line of diseases that have endangered the human race. No matter how frightening it is, it’s still pretty survivable. All you need to have is the right knowledge to win the battle.

This is, needless to say, a gigantic problem in accurately diagnosing Swine Flu-which is critical to beginning appropriate treatment as well as initiating infection control procedures and public health measures.
Currently, accurate testing for Swine Flu and other vial infections that can cause similar symptoms is difficult and requires specialized testing. This testing is time consuming and only available in specialized reference laboratories, which are few and far between. Fortunately, this may change in the near future.

This is extremely important in managing patients, communities and economic issues. Other tests have lower clinical sensitivities and specificities which can lead to both false positives and false negatives.
By incorrectly identifying an outbreak as due to H5N1, patients could be treated with anti-virals unnecessarily (thus raising the chances of selecting for drug-resistant strains), unneeded vaccination programs in asymptomatic individuals could be initiated, and damage to local and national economies-through such measures as culling of commercial poultry operations-could occur. Conversely a false-negative test result could lead to catastrophic outcomes, including a pandemic.
The assay utilizes swab specimens taken from the nose and takes less than 5 hours to perform. It was developed for use as a diagnostic tool and for infection control in hospitals. The assay has been slated for expedited review by the Food and Drug Administration (FDA), and TM Bioscience is hoping to have an FDA-cleared test available by the end of January 2006. If this assay delivers what it claims, it will have huge applicability on a global basis and save many lives.

There also antiviral drugs available for treatments of influenza, the virus is resistant to amantadine and rimantadine but sensitive to the oseltamivir and zanamivir. There remains concern that this strain may mutate develop resistance to oseltamivir in the future. For the treatment and prevention of the Swine Flu, the CDC recommended the use of Tamiflu (oseltamivir) and Relenza (zanamivir). Studies shows that the medication continues to maintain its effective that why the U.S. government had already extended the shelf life of federally stockpiled Tamiflu from their original five years to seven years.

At its basic foundation, family systems theory holds that clients with any mental health diagnosis, addiction or substance abuse disorder, can be more effectively treated if we take their relationships with their families into consideration. The client’s family system, be it functional or dysfunctional, is an important component to address when treating someone in recovery. Without information on the family history and dynamics in which an individual is a part, treatment is incomplete. While family work clinician’s focus is on the family unit or system, they do not de-emphasize the importance of dealing with the internal states of the individual. Family system’s approaches broaden the scope of assessment and interventions to provide best practice methods for their clients.
In assessing a client’s presenting problems, a family system’s oriented practitioner will be interested in a person’s current living situation, life experiences and what the client sees as his/her problem. As an example, here’s how therapists may initially assess from a focus on the individual versus an assessment from a family systems perspective.
Maria has come to an agency that provides services for recovering alcoholics. A counselor in individual therapy may inquire about Maria’s perspective or cognitions about her alcoholism. A therapist from a family or systemic orientation would not only examine Maria’s self-identifications but her cultural and intergenerational family experiences around alcohol as well. The individual- focused therapist would begin therapy with Maria alone. A family systems counselor may include the client’s parents, siblings or any other person(s) Maria may want to involve. Overall, the family-oriented therapist will want to explore how systems, from a micro to macro level, may be a part of Maria’s perception and hence problem with alcoholism. This enables the practitioner to find explanations beyond the client’s personal identification with their substance misuse. The therapist will want to understand how Maria’s alcoholism affects the family and how it is or was integrated into and is being maintained by family relational patterns. This broad look into Maria’s life will enable the counselor to provide a more unified and effective intervention plan.
While there are many styles or schools of thought and practice in family systems therapeutic interventions, most experts will agree that although they may operate or approach “family work” differently, practitioners should use the approaches that best benefit the client. Clinicians should also consider their comfort level and expertise in choosing a method of family therapy. The psychodynamic, intergenerational and family-of-origin approaches focus almost exclusively on the client’s past. Strategic, Structural, Cognitive-Behavioral and the Psycho educational modalities deal with the present and its influences on the client with little thought to the past. One of the most familiar and focused on the here and now methods of family systems therapy is the Experiential-Humanistic approach.
Experiential therapeutic methods are implemented so that a client can explore actualization, choice, freedom and growth. The focus is on the present interactions between the family and the therapist as opposed to exploring past experiences. The therapist’s characteristics and sense of self is an integral component in using this approach. Growth is assumed to be a natural process and that painful experiences are a part of life. Experiential family therapists (EFT) working with clients suffering from drug addictions or substance abuse, believe that these dysfunctional behaviors are actually the failure of a person to realize their potential for personal growth.
The goal of the EFT is to assist clients towards growth of self. By working towards spontaneity, bringing their creativity to light and learning how to play again, the client will hopefully begin to realize their potential. It is in engaging the families in the therapeutic process that will help you gain a better insight into your client’s mental health or substance abuse disorder and guide you to employing the most effective interventions. The recent hurricanes Katrina and Rita have been powerful reminders of how destructive the forces of nature can be, and how preparation for them can mitigate their effects. Avian influenza, commonly referred to as “bird flu,?is a powerful force of nature that we must prepare for-or suffer the potentially devastating health and financial consequences. Swine Flu is a viral contagious disease, just like the regular seasonal flu, but it might turn out to be 70 times more deadly. You must take the initiative to prepare yourself and your family for the coming bird-flu pandemic.
There are four essential areas that you must address to prepare for the bird-flu pandemic: 1) “social distancing? 2) commodities-including food, 3) personal protective equipment (PPE), and 4) financial preparation. Social distancing refers to your living and work situations when the pandemic strikes. Without going to extremes, you want you and your family to be as far away from other people as possible. Swine Flu is just like the regular seasonal flu in that you become infected from other people, not birds. (Although it might be possible to acquire the viral infection from birds, it is much more likely that, if you do become infected, you will have acquired the virus from another person, not a bird.)
The bird-flu virus is extremely contagious; it is transmitted though casual contact with a contagious person (who might not have any symptoms during the first 24 hours of infection), through touching contaminated objects, and through the air. Because of this, you want to stay away from people as much as possible, and that means spending more time at home. Your children will not be at school, they will be home. If your home is on the 73rd floor in an apartment building in New York City, how are you going to avoid other people? You might want to think of an alternative living situation for a few months.
The same principle applies to your work setting. If you can telecommute, that is the best scenario. If you don’t telecommute now, but because of the type of work you do it might be a possibility, discuss it with your employer. If you will have to continue to work closely with others at your job site, what can be done there to help protect you and others from infection? How can policies and procedures be amended to minimize contact with coworkers or customers? Are there hand-washing stations available? What are your employer’s plans for dealing with the coming pandemic? Discuss these and related issues with your employer and coworkers.
The second area that must be addressed is “commodities-including food.? There will be sporadic difficulties manufacturing or producing goods-because workers around the globe will be sick or otherwise absent from work. There also will be supply chain disruptions-both because workers will be sick or otherwise absent from work, and because of regional, national and/or international restrictions on travel. These problems will cause a decrease or the unavailability of most or all of the products we easily have access to now.
Commodities such as soap, toothpaste, toilet paper, and virtually everything you can buy at stores such as Wal-Mart will be difficult or impossible to obtain-for periods of weeks or months at a time. This includes the most important commodity-food. The federal government is always telling us to stock up on emergency supplies for three days. This will not be sufficient preparation for the coming deadly bird-flu pandemic. There will likely be limited food available in stores. In addition, stores are places you want to avoid anyway, because potentially contagious people might be there. Stock up now so that you have sufficient commodities, including food, for a period of months.
The third area to address is so-called personal protective equipment (PPE), which you will have to use, depending upon circumstances. PPE includes special face masks, called N95 respirators, which help prevent infection through inhalation of the virus. Remember that avian influenza (“bird flu? is a very contagious disease that can be transmitted through the air. The only way to counter this source of infection is through the use of special N95 respirators. These are disposable face masks that can be worn for up to eight hours.
N95 masks were the type of masks worn by hospital workers during the 2003 severe acute respiratory syndrome (SARS) pandemic that struck a number of cities around the world, including Toronto, Canada. Surgical masks or other common face masks, sometimes used when sanding or painting and so forth, are not effective.
Other elements of PPE include disposable latex or vinyl gloves, eye goggles or face shields, gowns impervious to liquids, and sometimes disposable booties or disposable head caps. Keep in mind that during the pandemic, most people who become infected will have to be cared for at home, not in crowded and overwhelmed hospitals. This means that caregivers taking care of loved ones at home need to be protected from the virus, just like hospital workers working in hospitals. The only way to be protected is to wear PPE. (Just washing your hands-the federal government’s primary recommendation for caregivers at home-will not be enough.) Once the pandemic starts, demand for PPE will be huge and supplies will be in very short supply-or nonexistent. Buy now or suffer the consequences later.
The last area that must be addressed before the bird-flu pandemic strikes is personal finances. This is an area that governments at all levels have been mute on. However, preparing your finances to sustain yourself and your family during (and after) the pandemic could prove to be the most important area of preparation. Although the bird-flu virus is deadly and many of us will fall ill, most of us will not die from it-only one to two percent of the population will likely die. The vast majority will live-but under what circumstances?
Think of hurricane Katrina-where most people survived-but where hundreds of thousands are now homeless and underemployed or unemployed. Because of the potentially severe local, national, and international economic consequences of the bird-flu pandemic, many of us will suffer financially. Businesses around the world will not be able to make or distribute products or provide services. There will be layoffs and some companies will go out of business altogether. At a minimum, people will be out of work for periods of weeks or months. Your child or children, if you have any, will be at home-not in school or day care. Will that force one parent to stay home from work to care for them? How will you pay your rent or mortgage and your bills under these circumstances?

When it comes to bird-flu preparation could make the difference between life and death, how much you and your family eat, and whether or not you can pay your bills, including your rent or mortgage.
